You will create an individualized plan of care for your client with a focus on identified health problems, health promotion, disease prevention, and referrals. Referrals must be from your client’s community and must have names, websites, and referral information listed so that the client would know how to make an appointment. First, complete chart (plan of care template). Then write a two-page paper summarizing findings, discussing priorities, and how they were determined for the plan of care.
