You are rounding in an inpatient psychiatric hospital and are meeting Mr. J. for the first time. He is a 67 year old widower whose wife passed away about 9 months ago. Mr. J. was admitted late last night after being observed outside his home drunk, waving a gun around and yelling “I’m just done with it all!”. Police were called by his next door neighbor and he was brought directly to this facility. Mr. J. tells you that he didn’t really mean that he wanted to die and he had no plans to hurt anyone but that he was drunk and upset with his neighbor. He reports that he tends to “fly off” when he drinks. He states that for the past several months he has been drinking at least a fifth of whiskey every 2 days. He states that it helps him to get through the days. He lives alone and his main social contact is going to a local bar and drinking with a small group of regulars there. He has an adult daughter who lives about 5 hours away. He has no other family. His last drink was about 16 hours ago. The longest time that he can remember being sober in the past several months is 2 days.
