This week we will be using the Power Point from last week’s Physician Documentation for Billing and scroll to slide 6-Definitions of the Elements of the HPI. You will be using this grid as you read through each of the four HPI (history of present illness) scenarios and circle statements, highlight or write the words at the bottom of the page with the correct classification for the sentences or words that describe any element found in that paragraph. For example: “chest pain since 11am” you will identify CHEST as the LOCATIONA of the pain and SINCE 11AM as the DURATION. This would count for the HPI as 2 elements for your auditing of the paragraph. To assign the highest level of comprehensive for the HPI you would need to find 4 different elements (not 2 locations if it’s documented twice in the scenario).
