Medical Center East is a 500-bed acute care hospital that includes 4 medical and 2 surgical inpatient units. The hospital recently (within the last year) implemented a new Electronic Health Record (EHR) system and overall, the project has gone well with acceptable user adoption. The hospital’s annual safety and quality report indicates a 5% increase in patient falls, specifically in the medical surgical units. Currently nurses complete a Falls Risk Assessment in the EHR and based on the score patients are placed on falls precautions and patients at high risk are assigned a sitter. The expense to provide sitters for each patient is growing and the Chief Nursing Officer (CNO) knows that this model is not financially or operationally sustainable for the organization due to the lack of financial and staffing resources. The CNO has asked your help in researching technology solutions that would support the falls prevention program. She has expressed interest in a centralized virtual monitoring system in which staff monitor the patients from a central location versus having individual sitters for each patient. One option for this model would include a monitoring system with cameras in each patient room with bidirectional technology for communication and visual observation. The Chief Financial Officer (CFO) and Chief Information Officer (CIO) have endorsed the concept and intent, but they will not approve the project funding until a thorough assessment and vendor Request for Proposal (RFP) is complete. You are the project manager assigned to lead the project implementation of this new technology for falls prevention. As the project manager, you are responsible to ensure that the details outlined in the assignments are complete and presented in a professional manner. An initial analysis of the current state is your first assignment. You will utilize clinical and financial data to help justify the system selection and overall implementation. The reporting team has provided data for the 4 medical units and 2 surgical units that includes falls data and data regarding the nurses’ efficiency in using the newly implemented EHR. Nurse efficiency is measured in minutes per nurse. The more minutes in the record can indicate a lack of efficiency using the EHR. This would be an initial thought; however, as part of your analysis you would interview and observe the workflows that the nurses currently experience. The data set is included with your first assignment. An additional observation in analyzing the data would be to analyze the correlation between the nurse efficiency scores and the fall rates for those units. The CNO believes Medical Unit 4 might be the best unit for the pilot of the project based on the unit’s past performance.
