Concept Overview Worksheet – Health and Illness Concept: Perfusion Related Concepts: Gas Exchange, Comfort/Rest, Mobility, Nutrition, Patient Education, Inflammation, Clotting, Cognition Exemplar Presence in Curriculum Level 1: NSG 200: Assessment of the cardiovascular and lymphatic systems Level 2: NSG 170: Hypertension; Peripheral vascular disease; Pre-eclampsia Level 3: NSG 210: Coronary artery disease; Congestive heart failure Level 4: NSG 252: Shock; Multiple organ dysfunction syndrome; Arrhythmias; Congenital defects-infant/child Definition: The flow of blood through arteries and capillaries to deliver nutrients and oxygen to cells and remove cellular waste products. Scope: Ranges from optimal perfusion to no perfusion Central perfusion (mechanisms for blood delivery) and local or tissue perfusion (actual amount of blood available to target tissues) Physiological Processes and Consequences: Threats to central perfusion and cardiac output o Preload: Hypovolemia or hypervolemia o Contractility: Loss of the myocardial contractile force, either acutely, e.g. acute ischemia, or conduction abnormality, or from chronic changes, e.g. ventricular remodeling, endocarditis, pericarditis, and valvular disorders o Afterload: Increased or critically decreased systemic vascular resistance (SVR) Threats to local or tissue perfusion o Decreased cardiac output o Vasoconstriction or blockage of the arteries or capillaries supplying the tissue, e.g. atherosclerosis or thrombosis o Conditions that interfere with cellular gas and nutrient exchange, e.g. edema.
