Chief complaint: “I have back pain.” HPI: The patient states he was moving furniture two days ago when he felt sudden lower back pain, which has been present since the initial incident. He states it is worse in the morning and with flexion. While the pain is present throughout his lower back, it is prominent on his right side with numbness and tingling radiating down into the right leg. He denies any fever, chills, unilateral weakness or changes to bowel or bladder.
