He is seeking treatment to control anxiety and is also requesting an “as needed” medication for “when it gets really bad.” He does have a history of alcohol abuse while he was in high school. He reports he “does not drink like that anymore.” He denies any significant past medical history or allergies. His vital signs are all WNL.
