56-year-old female with exertional dyspnea and orthopnea Patient demographics student includes patients age and gender Chief concern Presents to the clinic today with complaints of exertional dyspnea and orthopnea. HPI Student includes a detailed HPI. This must include most of the relevant points relating to the CC. This also must include pertinent negatives relating to the CC. Physical exam Student must detail the physical exam performed that relates to the CC. They must not miss any key exams that should be performed for the chosen CC. Workup Student will detail any and all orders for diagnostic work up.
