Encourage active range of motion exercise Reposition every two 2 hours and PRN Assist patient with walking, if possible, by utilizing assistive device. Instruct in the safe use of mobility aids. Nursing Dx: fatigue R/T: imbalanced AEB: weakness Resident will be free of falls throughout the year. Make sure the call light is working and reachable and personal items within reach. Ensure the resident is appropriate foot wearing when ambulating or mobilizing wheelchair. Make sure the floors are free from spills and clutter, and adequate lighting. Encourage progressive activity and self-care when tolerated and assist as needed.
