Ms. Amy Scarlet is a 36-year-old female

Ms. Amy Scarlet is a 36-year-old female that is coming to her PCP for a follow up for back pain/weakness. Since the last appointment Ms. Scarlet had an MRI which revealed an L3 and L4 herniated disc. She has been going to the chiropractor twice a week with some relief noted but wants to obtain further treatment for changes in mobility. She has a history of prior herniation which resulted in a discectomy at the L5 level, obesity, anxiety, osteoarthritis of the back, depression and HTN. The patient works as a librarian and states that she often is bending, stooping, or picking up heavy books throughout the day. I have had to use a chair instead of standing when stocking books for fear I might fall. At this time the patient is not interested in surgery and wants to try non-surgical options for a while. Heath History and Physical Exam: Ms. Scarlet reports that she noticed some back and leg pain starting about 3 months ago but kept thinking it was just due to a fall she had around the same time. She has been using ice and heat to relieve the pain along with over-the-counter ibuprofen and Tylenol as needed. Ms Scarlet’s main concern now is the weakness in that leg. She states, “at times it seems as though it will not hold me”. The patient states “the most comfortable place is in her recliner and standing for more than a couple minutes causes her leg to feel weak”. Vital signs: BP 118/84, HR 88, Resp. 18, Temp. 97.8 orally. Weight: 277 lbs. Nursing assessment: Upon assessment the nurse finds 3/5 strength rating with dorsiflexion and plantar flexion on the left leg. Right leg is 5/5. Upon ambulation you note that her gait is slightly unsteady, and her hips are uneven with the left leg appearing shorter. When performing ROM the left leg has less mobility and at times you notice the patient grimace while performing the movement. The patient also was unable to feel a soft touch on the toes on her left foot. The right foot was WDL. Provider Plan: Continue heat and ice and over the counter meds for pain management PT consult with focus on back strength, core strengthening and left leg exercises Leg exercise pamphlet provided to patient with instructions to complete 2-3 times a day Pamphlet on safe lifting techniques provided along with information on weight management.
