Reply to this post giving an account of the 5 domains of social determinants of health According to Healthy People 2030, the five domains of social determinants of health are: Economic stability – people with jobs are more likely to not live in poverty and are healthier, which is why it is a focus of healthy people 2030. Education access and quality – people with higher education are more likely to be healthier and live longer, living in poverty and experiencing that type of stress impacts the ability to live someplace safe, get jobs, and they have more health problems. Health care access and quality – people not having health insurance prevents getting the care or medications that they may need. Lack of a primary care provider prevents preventative screenings. Neighborhood and built environment – low income areas have higher increase of violence and may have unsafe water or pollution. Safe neighborhoods increase health and quality of life Social and Community Context – relationships impact health and well being. Providing support to those in need can assist them in preventing negative well being. How do they affect the development of disease? Being at risk for poverty, low education levels, lack of healthcare access/affordability, safe neighborhoods and lack of social relationships put the person at risk for poor health outcomes and the development of disease (Healthy People, 2030). Healthy people 2030 has targeted these 5 areas to improve where the person lives, works and learns in order to provide them increased health benefits, longer lives, and support for the needs of the stressors they are experiencing (Healthy People, 2030). According to the CDC, you need three things for infection, 1) Source = where the infection lives 2) Susceptible person = must have a way for the germ to enter the body and 3) Transmission = a way the germ moves into the susceptible person. What are some ways community health nurses can break this chain? The best way to break the chain of infection is to wash your hands frequently, keep current on your vaccinations, cover your cough and sneezes, stay home when sick, use appropriate PPE, and clean and disinfect work and living areas, use safe injection practices and educate on all of the above continuously to patients, co-workers, family, and anyone who will listen (Infectionpreventionanyou.org, n.d.).
