Case Study Denise, a PHN in the Healthy Babies Healthy Children program, has arrived at her initial home visit with Lisa, a recent and young immigrant to Canada who has just given birth to her second child, a baby girl. She also has a 3 year old son. With the interpreter present, you begin the home visit by asking Lisa about her birth experience. She tells you it was an uneventful birth, “easy” and uncomplicated. You start to ask Lisa how she is coping with having two children under the age of three – she says things are “ok”. She tells you her older son is eating a restrictive diet and drinks 40 oz of homo milk per day. She tells you the “doctor says he is below the 5th percentile” for his age. You ask Lisa who she lives with, as you noted an elderly lady and man in the distance when you entered the home. She tells you she lives with her husband, and in-laws. You ask her if her husband has any concerns about the children, and she tells you that he is concerned that their son doesn’t say many words or uses complete sentences. You ask her if she is getting much support from her in-laws and she says “no, they are very old and can’t help much. They make more work for me.They are actually driving me crazy”. Upon further probing, Lisa reveals she hasn’t been getting along with her in-laws. She tells you that she is constantly cleaning up after them and they give a lot of unwanted and inaccurate advice. She says it’s been a strain on her marriage too; she has asked her husband if they can go get their own house together but he tells her he can’t do that because as the eldest child, he has a responsibility to his parents. He also thinks that she needs the extra help. Lisa has been increasingly frustrated by her living and life situation over the past few months and gets emotional. You ask her how she’s coping and she reveals that she isn’t doing well. She reveals that she isn’t eating well, and has difficulty sleeping. She says that she has thought of ending her own life, and taking her children with her. You ask her if she has a plan on how she might do that, and she tells you that she has thought of jumping off a building while holding the children, or stabbing them with kitchen knives when cooking dinner.
