Morphine sulfate 2 mg IV q1hr for moderate pain; may give 4 mg q1hr IV for severe pain Penicillin G 5 million units IV piggyback x1 now (this is the first dose), then 2.5 million units (per dose) q6 hr for a total of 48 hours Lispro insulin subcut before meals and at bedtime based on the following sliding scale: Blood glucose to 150: No coverage Blood glucose 151-200: 2 units Blood glucose 201-250: 4 units Blood glucose 251-300: 6 units Blood glucose greater than 300: Call provider
