37 y.o man with Wolf-Hirschhhorn syndrome who

37 y.o man with Wolf-Hirschhhorn syndrome who was admitted with influenza and respiratory failure which was treated as severe sepsis and a very stormy hospital course. Course was complicated by severe constipation secondary to a stool ball in the colon, possible stercoral colitis. Treatment for that consisted of flexible sigmoidoscopy, multiple enemas, eventually had improvement; however, this caused patient discomfort and was unable to void on his own.. He required intubation and reintubations multiple times; mainly due to his family feeding him his favorite foods which in their mind, made the patient happy. He underwent PEG placement and nutrition was initiated through tube feedings due to the patient’s inability to swallow. The patient is very fragile and after 25 days as a patient, family reports he is at baseline. 1. Why is this an ethical issue? 2. What options do you as a healthcare worker to assist with this patient? 3. Is the family causing patient harm by going against medical advice in feeding him when the patient couldn’t swallow and caused aspiration pneumonia and then required intubation?
